Community Outreach & Partnership Development

CHWs will also help PKHA build relationships within the communities we serve. This includes calling, emailing, and visiting clinics, medical offices, food pantries, churches, senior centers, community organizations, and other trusted community locations.

Outreach responsibilities may include:

• Introducing PKHA and its Medi-Cal Community Health Worker services
• Developing relationships with organizations that serve Medi-Cal members
• Identifying potential community referral partners
• Distributing approved outreach and educational materials
• Connecting interested and potentially eligible individuals with PKHA for screening and services
• Following up with community organizations and referral partners
• Participating in community events and outreach activities as needed

Requirements

• Current Community Health Worker (CHW) certificate
• Applicants without a CHW certificate must be willing to complete an approved CHW training program and obtain certification as a condition of continued employment
• Fluent in Spanish and English
• Strong telephone and interpersonal communication skills
• Comfortable communicating with Medi-Cal members and community organizations
• Strong organizational and documentation skills
• Reliable computer and internet connection
• Private workspace appropriate for confidential member communications
• Ability to work independently in a remote environment
• Ability to travel locally within Los Angeles County for community outreach
• California resident in Los Angeles, Long Beach, Lakewood, Sacramento, Fresno, Bellflower, Wilmington, Cerritos, Downey, Hawaiian Gardens

Location: Remote within Los Angeles County, CA, with local community outreach and occasional in-person visits required

Languages: We are currently seeking Spanish/English bilingual candidates, but CHWs who are bilingual in other languages are strongly encouraged to apply as PKHA expands services to diverse communities. Cambodian/Khmer, Chinese, Thai, Vietnamese, Korean.

Pay: From $19.00 per hour
